I authorize Painting With Pat, Inc. and its employees to take all necessary steps to insure my/my child's health and safety in case of an emergency. I agree to hold Painting With Pat, Inc. and its employees free and harmless from damage or injury sustained by participation in or the operation of it programs. In an occurrence involving a juvenile exhibiting undesirable behavior that is disruptive or impose on the rights of others, parents/guardians/school officials are responsible to neutralize the behavior. I also authorize Painting With Pat, Inc. to use my/my child's name/school's name/ and photograph for educational and public relations purposes related to Painting With Pat, Inc.
